Hormone Replacement Therapy…Still a Possibility!!!

In July 2007 the preliminary results of the Women’s Health Initiative (WHI) study caused many women to discontinue their hormone replacement therapy (HRT). Now, seven years later, the pendulum seems to be swinging back to center regarding the safety of HRT for menopausal symptoms. The data on women < 10 years since menopause (mp) who received HRT in the WHI study did not demonstrate and increased risk of heart disease, stroke or death. Although this does not address long term use, > 5.2 years, it does suggest that using the lowest dose for the shortest period of time may be a safe option for women with severe mp symptoms.

The risk of invasive breast cancer with HRT use received a lot of publicity when the WHI results were released in 7/2002. The small risk of breast cancer (8 more per 10,000 women) in women taking both estrogen and progestin was not detected in the subgroup of women who had never taken HRT before which represents the typical patient seeking HRT. And…to put this into perspective… the risk of breast cancer in women who are obese is 2.5 times greater than those who are taking combined HRT. Obesity actually carries a greater risk for breast cancer than HRT use.

Current ongoing studies are investigating the benefits HRT may have on cardiovascular disease and Alzheimer’s disease in women who are newly menopausal. Results from these studies will be available in 2010 and 2011.

Flu Vaccinations

Dr. Fisher is not offering flu vaccinations at her office but she strongly recommends that her patients who are at high risk for the flu or H1N1 consult with their primary care physician regarding guidelines for vaccination. It is especially important for pregnant women, people ages 6 months-24 years, parents/care providers of children 6 months or older, and medical personnel to get vaccinated.

Focus On Sun Damage...Fall Special

Photo aging includes skin changes seen with aging plus changes seen with sun exposure. These include excessive wrinkling, pigmentation changes, and a yellowing appearance to the skin. Common pigmented lesions of cosmetic concern are:

• Solar lentigines- Also called sun spots or liver spots these pigmented areas usually occur
  on the face or back of hands.
• Melasma- Another form of hyper pigmentation of the face most commonly seen females.
  The darkened areas tend to occur on the forehead, upper lip, and cheekbones. The areas darken with sun exposure and lighten without. This condition may be increased in pregnancy and with hormone use. Sunscreen is a must to prevent recurrence.
• Ephelides- Commonly called freckles, these areas of increased pigmentation are of no
  concern and tend to come and go with sun exposure.

Many products and procedures exist to reduce and eliminate the signs of photo aging. Pharmaceuticals change the skin biology by remodeling the epidermis creating a more uniform color and texture. Commonly used pharmaceuticals are retinoids. These products are approved by the FDA. Cosmeceuticals are made by cosmetic companies and they do not change the biology of the skin but they may improve the appearance of fine lines. Commonly used products are topical antioxidants like vitamin C and E, fruit acids, and retinol. These agents are not FDA approved, are less expensive and less harsh on the skin than pharmaceuticals.
